TRICARE Mental Health in DFW: Do You Need a Referral, and Who's Actually In-Network?

Here's the sentence that surprises almost every military family we see: for outpatient mental health care, you do not need a referral — as long as you see a TRICARE network provider.

Not with TRICARE Select. And for most visits, not with TRICARE Prime either.

Yet every week we talk to a spouse in Fort Worth or a retiree near Arlington who's been putting off therapy for months because they thought they had to fight through a referral first. The confusion is understandable, TRICARE's referral rules for other specialty care are real — but for mental health, the door is more open than most families realize. Here's how it actually works in 2026, straight from TRICARE's own appointment rules.

The referral rules, plan by plan

TRICARE Select: no referral, ever, for outpatient mental health. Find a network psychiatrist or therapist, book, go.

TRICARE Prime: you can see a network mental health provider without a PCM referral for your first eight outpatient visits each year. After eight, you'll need a referral to keep your Prime cost-share. Two exceptions need prior authorization from the start: psychoanalysis and outpatient substance-use treatment. The full rules are on TRICARE's referrals page.

Active-duty service members: you're the exception. You still need a referral and authorization for civilian care — start with your PCM or your installation's military treatment facility.

The trap to avoid on any plan: seeing a non-network provider without a referral. That's the "point-of-service option," and it means dramatically higher out-of-pocket costs. The whole game is finding someone genuinely in-network — which brings us to the real problem.

The real problem isn't coverage. It's finding someone who takes it.

TRICARE's mental health coverage is some of the best in the country. Psychiatry, therapy, medication management — covered. The catch is that many private psychiatrists in DFW don't accept TRICARE at all, and the directories are full of profiles that haven't been updated since the provider stopped taking new patients.

The transition out — retirement or separation, when structure disappears and the identity question gets loud. Spouses running a household through a deployment on four hours of sleep. Teens on their fourth school in six years. That last part is the point, actually. You shouldn't have to translate your life to your psychiatrist.

VA care and TRICARE aren't either/or

A common misconception among retirees: that using VA mental health care means you can't use TRICARE, or vice versa. They're separate benefits, and plenty of veterans use both VA for service-connected conditions, TRICARE for faster access when VA wait times stretch. If you're a retiree with TRICARE and the earliest VA appointment is in October, you don't have to wait until October.
