Request the application from the coordinator for your campus
Cisco lists one Disability Services contact for the Cisco campus and another for Abilene and dual credit. Students email the applicable coordinator to request an application.
Identify your campus and enrollment arrangement in the request. Ask which application version and return method to use, especially if you attend courses in more than one setting. Begin early enough to discuss missing information rather than assuming an advising appointment also starts this process. Keep the coordinator’s response so you can follow the instructions for your own request.
Share Cisco’s documentation instructions with the relevant professional
Cisco’s published instructions request documentation from the previous five years on professional letterhead, signed and dated. They exclude documents written on a prescription pad.
Review the full college instructions with the professional who holds the relevant information. Ask the coordinator what to do if your existing documents do not match the listed format. Confirm the approved delivery method before sending personal material. Do not assume that a visit receipt, prescription or older school arrangement automatically meets the college’s requirements; let the coordinator explain what remains incomplete.
Track your review and the coordinator’s delivery to instructors
After the completed application, Cisco describes a coordinator discussion, preparation of an Accommodation Agreement for the student’s approval and then delivery to instructors.
When you receive the agreement, read it and raise questions before approving it. Ask how you will know the final version has reached the relevant instructors and what to do if your course schedule changes. Keep the agreement and the contact information available for follow-up. Submitting an application is one stage; it does not by itself establish that an agreement has been approved or distributed.
Contact the coordinator again at the start of each enrolled semester
Cisco instructs students to contact Disability Services at the beginning of each semester in which they enroll. The same page describes personal counseling as a separate, limited service.
Put the semester contact on your registration checklist and tell the coordinator if your campus, classes or circumstances have changed. Ask which information needs updating and how the current agreement will reach instructors. If you also want personal counseling, arrange that request separately. Academic paperwork, counseling and any outside clinical care can involve different people and should each have a clear next step.
Considering therapy or medication management at Lyte
Lyte provides therapy, psychiatric evaluation and medication management. Ask which service and clinician fit your needs. A medication evaluation does not guarantee a prescription or continuation of a particular medication.
Lyte Psychiatry offers telehealth visits to patients who are physically located in Texas at the time of the visit. In-person appointments take place at Lyte’s Pantego clinic; this campus guide does not describe a Lyte office at the institution.
Coverage and costs depend on your specific plan, clinician, visit type and deductible. Confirm network status and expected costs before booking. A student plan, parent’s plan or campus referral does not by itself establish Lyte coverage.
Confirm appointment format, availability and costs before booking. For questions, call 469-733-0848. Clinic hours are Monday–Friday, 9:00 AM–5:00 PM Central Time. Weekend appointments are limited and offered case by case.
Send medical records and insurance details only through the practice’s approved secure intake process.
Questions before you arrange care
Does Cisco automatically implement my agreement every semester?
Its published procedure tells students to contact the coordinator at the beginning of each enrolled semester. Ask what updates and distribution steps apply to your new schedule.
Who sends the approved Accommodation Agreement to instructors?
Cisco describes the coordinator sending it after the student approves it. Ask how to confirm delivery and handle later course changes.
