You filled the prescription three days ago. You feel exactly the same, except maybe a little queasy, and the internet is full of people saying it changed their life and people saying it did nothing. Both are telling the truth. They were just standing at different points on a timeline nobody drew for them.

Weeks 1 and 2: side effects arrive before benefits
Escitalopram (Lexapro) starts changing serotonin signaling within hours, but the mood benefit depends on slower downstream changes in the brain. What you may notice first is the body stuff: mild nausea, headaches, sleep changes, sometimes a temporary edge of restlessness. Most of it fades within two weeks. What you might also notice early: sleep and appetite steadying, a slightly longer fuse. Weeks 3 and 4: the quiet turn
This is when most people who respond start to feel it, and it rarely announces itself. Patients almost never say "my depression lifted." They say the shower stopped feeling like a project, or they answered a text the day it arrived. Weeks 4 to 8: full effect, or time to adjust
A full trial at a given dose takes about four to six weeks, sometimes eight. If you're clearly better, the job becomes protecting that gain. If you're only somewhat better, the usual move is a dose adjustment, not a new drug. And if nothing has moved by week six at an adequate dose, switching makes sense, and the odds are still good: many people who don't respond to their first antidepressant respond to the second. What can sabotage the timeline
Three things stretch this timeline out or fake a failure. Skipped doses, because steady blood levels are the whole mechanism. Alcohol, which works against the medication night by night. And quitting during week two because the side effects came before the benefits.
