Separate the family application from the caregiver route
VA’s ordinary CHAMPVA family application uses Form 10-10d. Eligible primary family caregivers follow the PCAFC route; VA describes their CHAMPVA enrollment through that program rather than the ordinary family application.
Before collecting documents, write the applicant’s role at the top of your checklist. Ask VA to resolve an uncertain route so the same paperwork is not sent to two different processes.

Check what is required and what is optional
The family instructions distinguish optional spouse or marriage evidence from required documents in situations such as adoption, stepchildren and school attendance for certain older dependents. Use the instructions that match the applicant.
Make two columns: required for this application and optional supporting material. Do not delay a packet solely because you have treated an optional document as mandatory; ask VA when the distinction is unclear.

Keep insurance details together
VA asks for other-health-insurance information on Form 10-7959c and copies of the front and back of applicable insurance cards. Review the current Medicare-related instructions when they apply.
Check that cardholder names, identification numbers and coverage information are readable. Keep a copy of what you submitted so a later request for clarification can be matched to the original packet.

Track enrollment separately from an appointment
Use the current submission instructions and save a confirmation or mailing record. If VA requests another document, record the exact item and response deadline before sending a replacement packet.
For an upcoming visit, keep a separate list of questions for the practice: whether it accepts CHAMPVA assignment, who files claims and what payment it requests. An enrollment application alone does not answer those billing questions.

Your next-step checklist
- Confirm whether this is the family or PCAFC caregiver route.
- Use the current application and instructions.
- Mark relationship evidence as required or optional for this applicant.
- Gather other-insurance information and readable card copies.
- Save the submitted packet and track enrollment and provider billing separately.
Keep personal, insurance and medical information in the appropriate organization’s approved secure channel.
Common questions
Does every spouse need to attach a marriage certificate?
VA lists certain spouse or marriage documents as optional supporting material, while other family situations have required evidence. Check the current instructions for the applicant rather than using one checklist for everyone.

Does CHAMPVA enrollment mean a particular clinic accepts it?
No. VA advises asking the provider about CHAMPVA assignment and billing. Confirm those arrangements directly with the practice.
