Establish what is ending and what remains available
Ask the EAP counselor or administrator how many appointments remain under your actual benefit, whether a referral is recommended and whom to contact after the last session. Put the last scheduled appointment and the next administrative task on the same calendar.
Texas Health's EAP describes assessment, short-term counseling and referral services. Its detailed guidance distinguishes this work from long-term or specialized treatment and explains that community referrals may carry charges. That distinction is a useful question for any EAP: which parts are included here, and what would change at the next practice?

Make the referral request specific enough to act on
Tell the counselor what practical conditions matter to your search: location, appointment format, sustainable schedule, language needs and the insurance product you would use. Ask what type of service the referral is meant to provide. Do not assume a counseling referral is also an appointment for medication management.
Create a short contact log with the practice, date contacted, response and follow-up task. If the first options are unavailable or unaffordable, share those concrete obstacles with the referring team. Ask whether another search or resource is possible; the log helps avoid repeating calls that already failed.

Confirm the next payment arrangement
Ask the receiving office whether the appointment would use an EAP authorization, medical insurance or self-pay. These are questions to resolve, not interchangeable labels. If you hope to keep the same counselor, ask whether that is available under a new arrangement and what terms would apply.
For insured care, obtain the plan's Summary of Benefits and Coverage and use the existing benefits checklist for the exact clinician and service. The summary helps you understand plan terms; an appointment estimate still needs the planned visit details. Do not assume the absence of an EAP copay continues after the handoff.

Separate records delivery from appointment acceptance
Ask the next office which records it needs, where they should be sent and how to request the transfer from the current office. HHS distinguishes ordinary medical records from separately maintained psychotherapy notes. A request for useful treatment records does not mean every private note will be included.
Keep two confirmations: the office has received the requested material, and the appointment is actually booked. Write down whom to contact if either step stalls. Treatment questions during the transition belong with the treating clinician; this organizer does not establish interim care or a prescription arrangement.

Your next-step checklist
- Record your own EAP's remaining sessions and final scheduled contact.
- Describe the requested service and practical appointment needs.
- Check how the receiving appointment will be billed before accepting it.
- Ask what records are needed and track the receiving office's confirmation.
- Save the next appointment details and the contact for unfinished steps.
Keep personal, insurance and medical information in the appropriate organization’s approved secure channel.
Common questions
Will an EAP referral pay for outside appointments?
Do not assume so. Texas Health explicitly separates community-provider charges from its EAP benefit. Ask your own administrator and the receiving office what payment arrangement applies.

Can I stay with the same counselor?
Ask the counselor and EAP whether that option exists and how future appointments would be arranged and billed. A previous EAP appointment does not establish the next payment agreement.

Should I ask for every psychotherapy note?
Ask the receiving clinician which treatment records are useful. HHS treats separately maintained psychotherapy notes differently from the medical record, so they should not be assumed to be part of a routine records request.
