Ask the EFMP office which forms apply
The official enrollment process directs families to the military treatment facility EFMP office. Depending on the need, the packet may involve DD Form 2792 for medical information, DD Form 2792-1 for early intervention or special education, or both.
Describe the administrative task you need help with: first enrollment, missing information or an update. Ask which current forms and attachments the office expects before arranging signatures or requesting a large records transfer.
Separate the family’s information from the provider’s assessment
The process sheet assigns demographic information to the family and the remaining portions to the appropriate medical or educational provider. Applicable education paperwork includes a current individualized education program or family service plan.
Give each professional the current packet and ask about their completion process. Review the returned information for accuracy with them; do not write or alter a professional assessment yourself. The official forms page explains that the forms should be downloaded for desktop viewing if a browser only displays a waiting message.

Check who must sign before returning the packet
Military OneSource says the service member cannot sign automatically for a family member who is over the age of majority. Follow the applicable family-member signature instructions after the provider’s portion is complete and accurate.
Use a short handoff list showing the form, the responsible person, what remains incomplete and the return destination. Helping a spouse or adult dependent organize documents does not itself authorize signing for them.
Track completeness review and the program decision
The EFMP coordinator or service designee checks completeness and helps resolve missing information. The service enrollment office makes the enrollment decision; family support follows enrollment. The process also calls for later updates as needs change and at the required interval.
Keep the final packet and the decision together. Ask which office handles a correction or new information. EFMP enrollment does not itself guarantee a duty assignment, authorize a private clinical visit or establish that Lyte can complete a particular form.

Common questions
Can the service member sign every form for the family?
No. The official process says the service member cannot sign for a family member over the age of majority. Follow the applicable signature instructions rather than assuming the military relationship grants authority.
Is EFMP enrollment the same as a referral to an outside clinician?
No. This packet documents needs for the military program’s enrollment process. Any clinical referral, appointment or payment authorization must be arranged through its own process.
