Identify the benefit and the kind of claim
Check that the notice concerns Federal Employees Health Benefits, or FEHB. FEP Blue’s published instructions distinguish requests for approval before care from claims after care. They also separate FEHB, Postal Service Health Benefits and Medicare prescription-drug appeals; enrollment eligibility has another route.
Write down the service, service date if it occurred, claim reference and decision date. Mark whether you are disputing coverage for care or your eligibility to enroll. Use the notice to locate the corresponding brochure section. If the category is unclear, ask the plan to identify the applicable procedure before reusing another appeal form.

Prepare the written carrier reconsideration
OPM directs members to explain their disagreement to the plan in writing, using the brochure’s benefit provisions. Include the decision being challenged and relevant supporting information. Keep the request and response together so the next reviewer can follow what the carrier considered.
Make a short index: disputed item, brochure provision, explanation and supporting document. Ask the billing office to explain an unclear service entry; ask the treating office about relevant clinical evidence. Let those offices provide their own records or statements. Verify the reconsideration deadline and recipient in your plan’s current instructions, and save dated delivery confirmation.
Build the OPM request from the completed record
If reconsideration does not resolve the dispute, use the decision letter and brochure for OPM review instructions. FEP Blue’s example asks for reasons, supporting documents and copies of carrier correspondence, with each claim’s material identified separately. It also describes exceptions when required claims procedures were not followed.
Compare your file index with the requested packet. Add a separate row for anything still held by the plan or provider, and ask how to obtain it. If the plan has not responded or the process appears stalled, promptly ask the plan or OPM which next step and deadline apply. Do not assume waiting indefinitely preserves a filing opportunity.

Check representation, destination and follow-up
FEP Blue requires written consent for most representatives and publishes different OPM destinations for FEHB and postal-plan reviews. Use the instructions that actually apply to your coverage. OPM’s consumer-protection page directs status questions to the contact identified in its acknowledgment.
If someone will act for you, ask which authorization is required and include it through the instructed route. Before sending, check the recipient, identifiers, readable attachments and contact details. Retain the exact submission and acknowledgment. Record any request for more information beside its due date and assigned sender; an acknowledgment is not a coverage decision.

Your next-step checklist
- Confirm FEHB coverage and the pre-service or post-service route.
- Identify the disputed item and relevant brochure provision.
- Keep the written reconsideration and carrier response.
- Prepare the OPM packet using your own notice and brochure.
- Verify representation, destination and deadlines, then retain acknowledgment.
Keep personal, insurance and medical information in the appropriate organization’s approved secure channel.
Common questions
Can I send every federal benefit dispute to the same OPM address?
No. The published instructions distinguish several programs and issues. Identify the coverage and dispute type before using the destination in your notice or brochure.

Does this checklist decide whether the denial was wrong?
No. It organizes the contract provisions, correspondence and evidence for review. Ask the plan to explain its decision and use the applicable review process for an unresolved disagreement.
