Request the current application
HAAM’s page provides an application-request step and explains that the application is sent by email. Membership follows a calendar-year cycle, so confirm whether you are joining or renewing. Review the current residency, work and income criteria without assuming last year’s determination answers this year’s request.
Before requesting the link, choose an email address you can monitor and note whether you have used HAAM before. Save the request date. If the email does not arrive, use HAAM’s official contact route to ask about the application rather than starting several requests with different addresses.

Make your music evidence easy to verify
The membership instructions ask for musicianship evidence from the past 12 months, including public links that identify the applicant by name. Examples include releases, teaching activity, credits and work records. Check the exact current instructions for the evidence you plan to submit.
Test each public link in a browser where you are not signed in. Note the name shown, the date of the activity and what the item demonstrates. If your professional name differs from your application name, prepare a brief explanation and ask what supporting connection HAAM needs. A broken or private link creates a question even when the underlying work is real.

Clarify which appointment you need
HAAM distinguishes membership processing from insurance-enrollment appointments, including its work with Foundation Communities. Tell staff whether you are seeking membership, help with coverage or both. Follow the current appointment instructions for your situation.
Prepare two separate question lists. For membership, ask what is missing and how activation is confirmed. For insurance help, ask which documents the enrollment appointment requires. Do not treat a date on a program announcement as the deadline for your own insurance action; have the enrollment contact identify the applicable timing.

Confirm membership before requesting the next service step
After submitting, track any missing-document request and the membership response. If you want help finding mental-health care, state that navigation need clearly and ask what HAAM can arrange under your current membership.
Keep the membership response beside a separate appointment note. Record the receiving organization, the service requested and who will contact whom. Membership processing does not, by itself, confirm a particular clinician, treatment appointment or payment arrangement. Ask the relevant office to settle those details before relying on them.

Your next-step checklist
- Identify whether the request is a new membership or renewal.
- Request the application through HAAM’s current page.
- Test music-work links and check the name and date shown.
- Separate membership questions from insurance-enrollment questions.
- Save the membership response and the next service contact.
Keep personal, insurance and medical information in the appropriate organization’s approved secure channel.
Common questions
Can I use the same music proof for HAAM and SIMS?
Review each program’s instructions separately. HAAM’s list establishes what it asks for; it does not establish that another organization accepts the same evidence.

Does an application request mean my membership is active?
Keep the request, submitted application and membership confirmation as separate steps. Ask HAAM what remains outstanding and how it will communicate activation.
