Identify the complaint route and dates
OCR’s public portal separates civil-rights complaints from health-information privacy complaints. For a reported language-access problem, review the civil-rights route. The current complaint package includes language within its national-origin category. A disagreement about a bill or a workplace issue does not automatically fit this process.
Record both the event date and when you learned what happened. OCR’s instructions generally require filing within 180 days of that knowledge. It may extend the period for good cause; ask OCR about your circumstances instead of assuming an extension. Keep the dates visible while gathering documents.

Prepare a specific account and supporting documents
Write the organization’s name and contact details, what occurred, when it occurred and why you believe language was connected to the problem. Describe your own experience in plain terms. A short chronology can separate a request for assistance from the response you received and the later effect on arranging care.
The form asks about witnesses and any efforts already made to resolve the matter. Include relevant correspondence or other documents, and explain what each attachment shows. Answer accurately if you have not tried another process; the form’s question is not an instruction to invent an earlier grievance.

Read the consent before signing
Read the complaint package’s consent and privacy explanations. OCR may need to disclose identifying information during its review. Consent is voluntary, but refusing it can hinder an investigation or lead to closure. Ask OCR about a concern before making your selection; filing does not promise anonymity.
If you are helping someone else, identify that person and your relationship. OCR generally needs the person’s signed consent unless an exception applies. Provide supporting authority documents when applicable and let OCR explain what it needs; being a relative does not settle that question.
Use the online route and keep the response
Start from OCR’s official complaint page and follow its portal link. Complete the complaint details, electronic signature and consent step, then retain the available copy. If you need another submission method or help in your language, call OCR at 1-800-368-1019 to confirm the current instructions.
OCR first determines whether it has authority to handle the complaint. An investigator may request documents or interviews, and OCR describes sending a decision letter after investigation. Keep the submitted version, any reference number and subsequent requests together. Follow the instructions in the actual response rather than treating submission as a finding.

Common questions
Can OCR help me communicate during the complaint process?
OCR offers language assistance for its matters without charge and describes accessible formats and services. State the language or format you need when asking for help.

Will filing arrange my next mental health appointment?
The complaint process reviews a reported problem. Continue arranging appointment logistics through the practice, and ask OCR what its review can address. A submitted complaint does not confirm an appointment or guarantee a particular outcome.
