Identify why you are requesting continued care
The HealthSelect referral page describes a possible temporary continuation when an ongoing treating provider leaves the network, subject to eligibility review. The linked form also includes a request type for someone new to HealthSelect who is receiving care outside its network.
Keep the provider’s network-change notice or your new enrollment information available, if applicable. Ask a Personal Health Assistant which request type and submission route you should use. A long relationship with a clinician does not by itself establish approval; the plan must review the actual request.

Prepare the clinician and visit information
The form asks for provider or facility contact details and the dates of the last and next visits. It provides room for more than one provider. Prepare accurate information for each clinician whose continued care you are asking the plan to consider.
For example, therapy and medication appointments may involve different offices. Give each its own row in your preparation notes and ask whether both should appear on the request. If a next visit is not booked, ask the plan how to complete that field rather than inventing a date. Check the provider’s mailing and contact details directly.
Coordinate the treatment-information and records questions
The form includes a medical and mental-health information section and an authorization concerning records needed for the decision. The plan may need information from the treating provider. Ask your clinical office which information it can supply and how the plan should request it.
Read the authorization before signing. Ask the plan to explain the requested records, recipient and purpose if any part is unclear. Use the official submission method and keep sensitive documents out of general appointment-search messages. The administrative task is to get an accurate request to the decision-maker, not to write your own medical eligibility finding.

Track submission and read the written determination
The form says a formal written decision will be sent. Keep the submission date, a copy of the request and any confirmation. Ask the plan what to do about an upcoming visit while the request is pending; do not assume the submitted form has changed how that visit will be paid.
When a decision arrives, ask the plan and practice to explain which provider, services, dates and payment terms it addresses. If the result is unclear or unfavorable, request the applicable next-step instructions. Keep the result separate from any appointment confirmation so a booked visit is not mistaken for an approved benefit.

Common questions
Does sending the continuity form guarantee in-network payment?
No. The plan decides the request. Ask about the specific upcoming appointment while review is pending and read the written determination when it arrives.

Can the request include both my therapist and prescribing clinician?
The form has space for multiple providers. Ask the plan how to include each relevant provider and what separate information is required; inclusion does not establish approval.
