Use your county for eligibility, then compare provider options
NTBHA’s directory covers Dallas, Ellis, Hunt, Kaufman, Navarro and Rockwall counties. It says people eligible for NTBHA services may choose among its listed providers across the region, regardless of their home county within that region.
Start by telling the access team where you live and the service you need. Do not infer eligibility from the nearest clinic’s address. A directory may include a provider outside the region; that does not automatically expand the residence rules or mean another county authority uses the same funding arrangement.
Compare service fit before distance alone
The directory separates providers and service descriptions and supplies an interactive map. Listings can include different age groups or kinds of support.
Make a short comparison with the clinic name, service sought, location, intake route and the answer received. Ask whether the listed office handles ongoing therapy, psychiatric evaluation, another support service or a combination. Record “not yet confirmed” when an answer is missing; a map marker is not evidence of an available appointment.
Use the appropriate care-coordination contact
NTBHA gives Dallas County a community-care coordination number, 800-241-8716. For Ellis, Hunt, Kaufman, Navarro and Rockwall, it lists 469-780-9159. Those contacts are distinct from asking an individual clinic to schedule a visit.
Describe the practical obstacle when asking for coordination: you cannot identify the appropriate service, the suggested office has not responded, the location is inaccessible, or the intake instructions are unclear. Offer the names and dates of previous contacts. Ask who will take the next action so that two offices do not each assume the other has completed it.
Verify the financial arrangement with the actual service
NTBHA describes pharmacy support for qualifying people using network prescribers, alongside a separate prescription-assistance program. That description should not be extended to every outside prescription or private practice.
When you reach a clinic, identify the funding or insurance question separately from the clinical request. Ask which assessment determines your responsibility, whether there is paperwork still required and which charges remain unconfirmed. If you also see an independent clinician, ask both offices how that arrangement affects the intended referral before assuming the programs share benefits.
Check that the referral became a next step
Use the NTBHA directory and care-coordination route as the starting point for a contact record. Distinguish an office suggested by the authority, an inquiry sent to that office and an appointment actually confirmed. Include the date and the person or team responsible for any outstanding record request.
If the clinic cannot meet the request, return to coordination with the specific reason and ask for another appropriate route. Keep medical details in the receiving organization’s designated process rather than a general referral-search message. For treatment questions while waiting, contact your current care team; this navigation worksheet cannot decide clinical urgency or replace an urgent service.
