Identify SSA-454 or SSA-455 from the notice
SSA says it mails a letter when a continuing disability review report is due. Its Disability Update Report, SSA-455, is a separately named report used within the review process. Do not replace the requested form with another one simply because both concern disability.
Keep the notice, response instructions and any scheduled contact date together. If the form name or return route is unclear, ask the Social Security office handling the request. Completing a report is a step in the review, not a decision about whether benefits continue.

Gather the information needed for SSA-454
The SSA-454 instructions ask for healthcare-provider names, addresses and phone numbers for the past 12 months, along with medicines and other organizations that may hold medical information. The report also asks about specified work, education and support-service history.
Use appointment records, medicine containers or your own records to check names and dates. Follow each question’s time period. If something is unknown or does not apply, label it as the form instructs rather than inventing an answer; use the Remarks section when more space is needed.
Complete your report and follow the consent request
SSA-454 says not to ask the healthcare provider to complete this report. A friend, family member or SSA representative can help. It also says you do not need to obtain records from doctors or hospitals yourself: with your consent, SSA requests them directly using the details you provide.
Read any information-release document included in the notice and follow its instructions. If a helper completes the report, identify that person where requested and answer about the beneficiary. Ask SSA for assistance if you cannot complete the report or need language help.

Choose the available route and finish submission
SSA’s current client page describes online completion, uploading a completed SSA-454 PDF, or mailing or faxing the report to a local office. Its interactive online i454 instructions apply to eligible adults without a representative payee. Ask SSA which route is available if the expected link is absent.
For an interactive i454, complete required fields and retain the successful-submission receipt and response summary. Saving unfinished work is not submission. Changes after submission go through the local office. SSA says DDS may request more information or an examination before a decision is mailed; keep later requests with the original notice.

Common questions
Should my psychiatrist complete SSA-454 for me?
The form specifically says not to ask the healthcare provider to complete the report. You can ask another helper or SSA for assistance with your responses.
What if I need to correct a submitted interactive online report?
SSA’s i454 instructions say submitted responses cannot be edited directly. Contact the local Social Security office to request the correction.
