Start from StarCare’s own intake page
StarCare asks people seeking services to complete the forms linked from its intake page before attending open access. The link opens its DocuSign intake route.
Check that you are completing the forms for the person who will receive services. Set aside time to read them and keep a private list of questions. If a required field is unclear, or you cannot use the online form, contact intake for help. A saved draft, completed signature and submitted packet may be different stages; confirm what remains to be done.
Ask which original documents apply to you
The page requests originals for verification, including identity, birth, income, insurance, Social Security and guardianship documents. Ask staff which items are relevant to your circumstances.
Put original documents in a folder you can keep with you. Make a separate list of missing items so the call is specific: “I have identification but cannot find this other record.” Ask how staff want you to proceed. Do not assume that every adult must have guardianship paperwork or that an uploaded image replaces a requested original.
Confirm the current open-access window before travel
At the October 14, 2026 check, StarCare listed Monday–Thursday, 8 a.m.–2 p.m., at 1950 Aspen Avenue. Intake questions go to 806-740-1421 or 844-472-8810.
Before leaving, confirm that the session is operating that day and ask about arrival instructions. Account for transportation, work and anyone accompanying you. If you have a firm departure time, discuss it with staff at the beginning. Use the actual intake window, rather than general business-office hours, when planning the trip.
Ask what happens after your intake review
StarCare describes service authorization and case-manager assignment for people found eligible. Intake does not guarantee a particular treatment or prescription.
Ask for the next contact and any remaining action in writing if available. Clarify whether staff will call you or expect you to call, and how to report a changed phone number. Keep unresolved financial questions and clinical questions in separate notes so each reaches the right person. Do not treat a form submission receipt as the final service decision.
Your next-step checklist
- Open the current official form link.
- Check submission status and save questions privately.
- Confirm applicable original documents with intake.
- Verify the arrival window and next contact.
Keep personal, insurance and medical information in the appropriate organization’s approved secure channel.
Common questions
Do online StarCare forms reserve an appointment?
The instructions describe preparation for walk-in intake. Ask the team to confirm any appointment or arrival arrangement separately.
What if I cannot complete the DocuSign form?
Contact the intake team with the specific access or form question and ask for an available alternative. Do not send sensitive answers through a public website comment.
