Confirm which leave rules your employer is applying
The Department of Labor says an employee's group health coverage must generally continue on the same terms during FMLA leave as if the employee had continued working. This does not establish that every unpaid parental leave is FMLA leave.
Ask the benefits contact to identify the leave category and dates in writing. Keep the leave notice with the plan name, member information, and your next scheduled appointment. If paid leave changes to unpaid leave partway through the absence, mark that transition date separately rather than treating the whole period as one payment arrangement.

Build a small payment record
Create one row for each premium due date: amount requested, date sent, confirmation reference, and any unresolved question. Save receipts in the same private folder as the employer's instructions. If a payment appears missing, send the reference to the designated benefits contact and ask for written clarification.
Keep this record separate from appointment copays or other visit charges. The premium question belongs with the benefits administrator; a practice can explain its own billing process but should not be treated as the authority on your employer's leave payment arrangement.

Check the appointment against the coverage dates
Once the benefits contact confirms the arrangement, ask the health plan to verify coverage for the planned date of service. Give the practice the exact current plan information and ask whether it needs an updated eligibility check for that appointment.
A useful call note records the date of service discussed, the plan representative's reference number, and any action requested from you. Ask about the named clinician and visit type rather than only asking whether the practice 'takes' the insurer. These are verification questions, not a guarantee of payment.

Recheck when the leave or return date changes
DOL identifies circumstances in which the FMLA obligation to maintain coverage ends, including exhaustion of the FMLA entitlement and certain employment or return-to-work changes. Ask the employer what a changed return date or leave designation means in your situation before assuming the original coverage arrangement continues.
Bring the revised dates back to the plan and the practice's billing contact. If the issue is an actual coverage loss, ask the benefits administrator for the applicable continuation or enrollment information; that is a different task from paying premiums during an approved unpaid leave.

Your next-step checklist
- Confirm the leave designation and paid-to-unpaid transition date.
- Request the employer's written premium terms.
- Save each payment receipt and confirmation.
- Verify the plan for the scheduled appointment date.
- Recheck the arrangement if your leave or employment status changes.

Common questions
Does unpaid leave automatically mean my insurance stops?
No. DOL's FMLA guidance describes continued group health coverage under applicable conditions, with the employee's share still payable. Whether that framework applies to your leave is a question for the benefits administrator; unpaid leave by itself is not enough information.

Can I assume premiums will still come out of payroll?
Ask before the unpaid portion begins. DOL describes different payment arrangements for unpaid FMLA leave and requires advance written notice of the terms. Obtain the actual instructions rather than sending money through a method you used for a different leave.
