Begin with the VA team handling your care
For the community-care process described by VA, your VA health care team must approve care and provide a referral before you schedule with a community-network provider. Ask that team what it needs to review your request.
If you have a preferred provider, give the team the name and location. Ask for help if you cannot find a suitable network provider or the provider you want is outside the network. Do not treat a private booking as a way to complete the VA approval step.

Separate a referral from a scheduled appointment
After a referral, VA says you may schedule the appointment yourself or ask the VA team to do it. If you schedule it yourself, VA instructs you to notify the team within 14 days.
Record who will schedule, the provider contacted, the appointment date, and when you reported it to VA. If the provider offers a different clinician or location, ask the VA team whether the change fits the referral before treating it as settled. Keep one person responsible for closing this loop.

Name the contact for unresolved coordination
VA's care-coordination guidance assigns its community-care team roles in eligibility, authorization, scheduling logistics, and follow-up. Ask your local team which contact handles a missing document, a changed appointment, or a question from the receiving practice.
Use a compact issue note rather than starting the story over with every call: patient identifier through the approved channel, referral reference, appointment date, unresolved item, and prior contact date. Ask both offices what records or other materials are still needed and who is expected to send them.

Track the approval period during follow-up care
VA advises patients to track ongoing appointments and the approved period; it will not cover services outside the authorization's scope. More care may require a new referral. Keep the authorization end date and any stated visit limits beside the appointment list.
Before arranging another visit near a limit, ask who will request any additional approval and how you will know the request is complete. A clinician's recommendation for follow-up and an authorization decision answer different questions. This guide does not establish that Lyte or another named practice is a VA community provider.

Your next-step checklist
- Ask the VA team to review and refer the requested community care.
- Assign one person to schedule and report the appointment.
- Match the authorization letter to the provider, care, and dates.
- Keep the local community-care contact with your paperwork.
- Track ongoing visits and resolve additional authorization questions early.
Keep personal, insurance and medical information in the appropriate organization’s approved secure channel.
Common questions
Does finding a practice in a directory mean I can book under VA coverage?
VA's process still requires its approval and referral. Ask the VA team to confirm the route for the provider and service you want. A directory result does not replace the referral and authorization process.

What should I do if the letter and appointment do not match?
Contact the VA community-care team and tell the receiving office what needs clarification. Identify the specific mismatch, such as provider, service, or date, and ask who will resolve it. Do not assume a verbal appointment offer changes the written authorization.
