Understanding OCD
The engine of OCD is the relief cycle. An intrusive thought creates distress, a compulsion relieves it, and the relief teaches the brain the thought was dangerous. Every repetition strengthens the loop, which is why compulsions feel necessary and make things worse.
Many presentations involve no visible rituals at all. Mental reviewing, silent counting, praying to neutralise a thought, and repeated reassurance seeking are compulsions, and people with these themes often go years being treated for generalised anxiety instead.
Themes vary widely and can be deeply distressing: contamination, harm, symmetry, relationship doubt, and taboo sexual or religious intrusions. Distress about a thought is the opposite of intent, and clinicians who treat OCD know that.

What your first visit involves
- Identifying the specific obsessional themes, including taboo ones people rarely volunteer
- Mapping compulsions, including mental rituals and reassurance seeking
- Time spent daily and the resulting functional cost
- Screening for depression, tics, body dysmorphic disorder, and hoarding
First appointments run longer than follow-ups so there is time to take a full history. You leave with a working diagnosis, a plan, and a follow-up date rather than a prescription and no explanation.
Therapy, psychiatry, and combined care
- Exposure and response prevention, the first-line psychological treatment
- SSRI medication, often at higher doses and for longer than in depression
- Dropping reassurance and accommodation by family, planned deliberately
- Psychoeducation so intrusive thoughts stop being treated as evidence of character
- Referral to intensive OCD programmes when severity requires it
Therapy and psychiatry are delivered in-house by clinicians licensed in Texas and share one chart, so a prescriber and a therapist working with you are looking at the same notes. Many people do best with both.

Frequently asked questions
Why doesn't regular talk therapy work for OCD?
Because it usually provides reassurance, and reassurance is a compulsion. Discussing whether the feared thing might happen relieves anxiety briefly and strengthens the loop. ERP works by doing the opposite.
Can you have OCD without visible compulsions?
Yes. Mental reviewing, counting, silent repetition, and reassurance seeking are all compulsions. Purely obsessional presentations are common and are frequently misdiagnosed as generalised anxiety.
My intrusive thoughts are violent or sexual. Does that mean something about me?
No. The intense distress those thoughts cause is precisely what indicates they run against your values. Taboo themes are a well-recognised part of OCD, and clinicians who treat it are not alarmed by them.
How long does ERP take?
Structured protocols typically run twelve to twenty sessions, with meaningful change usually appearing within the first several weeks of consistent between-session practice.
