Start with the claim contact
ERS identifies TIPP self-service and administrator contact as ways to start a claim. A claim manager is assigned after intake. Ask that contact which claim, document version and submission route apply before sending a clinical form to a practice.
Keep the claim reference beside the contact name, the date you spoke and the next response you are expecting. Separate a question about the claim from a question about an appointment or your employer’s leave process. When you call, name the unresolved item: “I need to confirm who receives the clinician statement for this claim.”
Confirm the current form and signature requirements
TIPP’s behavioral-health webpage links a master plan that takes precedence over conflicting descriptions. The plan effective September 1, 2026 uses an Approved Practitioner definition broader than the webpage’s physician-only wording. Ask the claim manager to confirm the applicable requirements for your treating professional and claim.
Send the exact request to the practice before promising that it can be completed. Ask whether an appointment, additional records, a release or a separate paperwork process is needed. A clinician’s professional title alone does not confirm that the clinician can assess the requested issue or provide the requested statement.

Prepare the work facts without writing the clinical answers
The behavioral-health form has a clinician assessment and signature portion. Leave that assessment to the treating professional. You can prepare a separate factual note with your actual job duties, the dates you want the office to understand and the administrator’s questions.
For example, distinguish “my last completed shift was Tuesday” from a medical opinion about when you can work again. Mark uncertain dates as uncertain and explain any correction. Give the practice the relevant request rather than a preferred diagnosis, claim outcome or prewritten clinical conclusion.
Track each requested item through receipt
Use a short list with one line per requested document: who has it, where it goes, when it was requested and whether receipt has been confirmed. The current plan places responsibility for a complete supporting statement with the participant, even when the administrator requests it from a provider.
If an item is missing, ask whether it was never received, was unreadable or needs clarification. Those problems require different follow-up. Save the response and ask which deadline applies to the outstanding item; do not treat an upload confirmation as a benefit decision.

Your next-step checklist
- Record the claim reference and assigned contact.
- Confirm the current form, signer requirements and submission route.
- Ask the treating practice what it can provide and when.
- Keep work facts separate from clinical conclusions.
- Track receipt and unresolved requests for each document.
Keep personal, insurance and medical information in the appropriate organization’s approved secure channel.
Common questions
Does the title Attending Physician Statement mean only an MD or DO can sign?
Do not decide from the form title or older webpage wording. Ask the claim manager to apply the current plan’s practitioner definition to the specific statement and treating professional.

Does a TIPP claim complete my employer’s leave request?
Ask the employer’s leave contact what separate request is needed. Keep the leave instructions and claim instructions clearly labeled so neither office’s response is mistaken for the other decision.
