Confirm the review and aid year with Financial Aid
UTA’s current instructions identify the 2026–27 form for summer 2026, fall 2026 and spring 2027. Its medical-expense description requires bills paid in the current year totaling more than 11% of family income. Ask the office whether this review fits your circumstances and which expense period to document.
The forms index lists Special Circumstance, Cost of Attendance adjustment and dependency-change requests separately. Identify the request by name when contacting staff. Do not assume that a medical bill makes every financial-aid form interchangeable.

Gather the expense evidence and explanation
The Special Circumstance form asks for documentation explaining the medical expenses and receipts or proof they were paid out of pocket, rather than covered by insurance or placed on credit. Its calendar-or-tax-year wording makes confirming the requested period with Financial Aid especially useful.
Prepare the required typed, signed and dated one-page explanation. Match each receipt to the expense it supports. Ask the office how to handle missing proof before sending a partial packet, and keep a copy of the documents you provide.

Complete the financial and family information
The packet calls for signed 2024 and 2025 tax returns with applicable schedules for the relevant student, spouse or parents. Complete the form’s actual and anticipated 2026 income columns for your status, using zeros where instructed instead of blanks.
Attach the correct 2026–27 Verification of Family Members form. The dependent and independent versions use different support statements. Read the applicable statement, list names, ages and relationships, and use the stated July 1, 2026–June 30, 2027 period. Ask staff which version applies if you are uncertain.

Check signatures and send through Financial Aid
The Special Circumstance form requires the student’s handwritten signature and a parent’s handwritten signature when dependent. The family-member form also has its own signature requirement; check both documents.
UTA’s forms page links a secure financial-aid upload and lists in-person delivery at MavCentral, University Administration Building, Room B17, plus mail and fax options. Use the current office instructions and retain confirmation. Financial-aid documents should not go through the UTA Admissions Portal.

Monitor the request and arrange family access
Complete outstanding MyMav to-do items and monitor that list for additional requests during review. Keep a dated copy of your response to each request.
If a parent will discuss aid information without the student present, UTA asks the student to grant guest access. Use the university’s delegated-access instructions rather than sharing a student password. Permission to help gather receipts does not by itself establish access to the financial-aid record.

Common questions
Does an approved review guarantee more aid?
No. UTA says a review may not change aid eligibility, and additional funding is not guaranteed even if FAFSA data changes. Ask the office to explain the actual outcome before relying on it.

Is this the same as paying a clinician’s bill?
This packet requests a financial-aid review. It does not itself establish a payment agreement with a provider. Direct questions about a bill or its payment arrangement to the issuing office separately.
